Vivago solution helps the multidisciplinary team to make decisions when developing the care and rehabilitation plan for the customer, and to implement it both during the assessment phase and after discharge. For example, solution enables assessment units to plan and implement remote monitoring of changes in customers' health status and wellbeing. Measurement initiated during the assessment period provides information to the whole multidisciplinary team for example, on the development of the rehabilitation, customer's wellbeing and functional capacity.
